Our Style #5 Change Order Form
Unique features of Change Order Form Style #5 (vs. styles 1-4 & 6):
- Standard terms and conditions which are fully editable and include provision to charge interest - keep them as is or use them as a starting point while editing them or delete them entirely and enter your own

Does Your State Require You to Include Your License Number?
Some states require construction contractors to list their contractor license number on all of their paperwork, including their proposals/bids and change order submissions, while other states do not require that.
